I've driven some pretty respectable distances in heels. Not 6 inch stilettos mind you, but 2-3 inches anyway. One trick is to not keep your feet vertical, but to approach the accelerator almost sideways, with the ball of your foot near the top of the accelerator pedal. Another trick is to use cruise control if so equipped. The brake and clutch have not ever presented any problem, but then I don't ever ride them, like I must ride the accelerator pedal to maintain speed.

Driving in heels
I wear 3 and 4 inch heels all the time when driving with no problem. When I first started I had to find a position that was comfortable and the right position for the seat. I have found that I do sit a little different when driving in heels as opposed to wearing my regular shoes.
I tend to sit up a bit more and find that my knees are a little off to right instead of straight ahead. I have found that this works with both my automatic and stick shift. It took a bit more getting used to on the stick shift as the peddels flet different.
