Ah come on. He or She don't care. I told mine last visit (week ago). Did not matter to him. Before I have worn brown colored panties and was careful how I took them down for exam. Like somone said he's sticking him finger up my bottom.
Wore my Bra and Panties. No Biggie even in the VA. It's not like I am nuts. Always wear my Women's Jeans and Tops. Heck all I got. Have to go Nude if I did not wear them.
Remember they are Dr's and Medical People. Not Fashion Police.

Unless he's trying to figure it out or Gay? It's not where I look at Guys. At 51 I notice if they have beer belly or such. Beard if they have one to see if they look like Civil War Type or such. ie Throwback

Since my wife is in the medical field, I've heard many times about men wearing women's undergarments.
You can believe this: They've seen it all.
Doctors and Nurses are professionals, and (in my opinion) they could honestly care less about what you're wearing. Oh sure, you might get a raised eyebrow occasionally, but their demeanor and attitude should not change. You should still be treated with all the respect and courtesy patients deserve.
There are laws which prevent them from disclosing your personal information, too. So my opinion is, "wear what you want to."

My doctor knows all about me, and is really cool about it. He's use to seeing me in various degrees of feminine attire.
The real embarrassing moments for me are in the locker room at work. All of my co-workers know that I cross-dress. They are all cool with it. The ladies often bring clothes to me. I still would feel weird about other males seeing me in my panties.
