I agree with Jamie, I don't own any male underwear and wearing a bra and panty just feels normal. Actually, now that I think about it, the idea of putting on mens underwear seems a little disgusting to me, probably they would have boy germs.
But there are some times that I don't wear a bra for some reason, and then I feel a little strange, like something isn't right.

I have only been wearing female under garments this year, I did slip two day though and I can tell the difference. I underdress with panties, pantyhose and usually a camisole. I love the way it feels. I do like wearing a pant liner ( petti pants? ) the feel on pantyhose on makes me feel feminine. I am much more comfortable now about who I am.

I always wear ladies underwear, and I had them on when I was at the VA the other day. It was really funny because they had me do a full exam and I was wearing ladies underwear and didnt know that was going to happen. That made it fun and exciting. They also probably think I am crazy. Whatever

let's hope they were concentrating more on their work than on your panties. But no telling, were they particularily pretty panties that would make them jealous?
But even if they think you're crazy, it's OK, it just gives you leave to wear everything you want next time.
I haven't been to a doctor in years, but I don't think I would dress any differently to go there.

Some of my doctors have seen my panties; at least two know I'm trans. The technicians see me more often in my panties than the doctors I think.
If I know I'm going to the doc and drop my pants, I'm certainly going to wear a nice color or print. No reason to not look nice even if I'm just in my panties.
These people are there to help you. Why worry about what they are thinking as long as you get the proper care you are there to get? Shoot... maybe you are their only treat in an otherwise hard day. be nice to them; give them that treat

The male physician's assistant at my dermatologist was quite polite when speaking with me at a follow-up visit last week. I had kept on my bra beneath the gown because they were checking my healing progress elsewhere. When I had to remove the bra so he could check the moles on my back and chst, he asked how long I had been pursuing transition (estradiol and cyproterone were listed on my medications,) and we discussed it for more than five minutes more. It was a very pleasant and professional conversation.

This is a bit like a situation I have so far only anticipated, if asked "are those ladies trousers (or shoes, or top or..)" I would answer, "no they're mine" and leave the rest open. One day I was asked if I was a cross dresser I replyed that no I was quite happy about it, a few years later I still wonder what prompted the question.
